Hospice care allows people who are nearing the end of their lives to remain at home or at the location of their choice, among family and friends.
Terminally ill patients and their families prepare to face the challenges of a journey through life’s final stages. Families and their loved ones establish relationships with a team of trained physicians, nurses, social workers, bereavement and spiritual counselors, all of whom are involved in providing comfort and care.
A Palliative Care program offers expert treatment of pain and continuity of care in a private residence or facility. A highly trained staff of nurses, home health aides, spiritual counselors and social workers are dedicated to offering expert assistance with symptom management. If needed, a transition to Hospice is available.
Bereavement services are an essential and intricate part of the Hospice program. Sivitz Hospice is committed to supporting families during the grieving period following the loss of a loved one. Bereavement support is tailored to meet the unique needs of each family. We are committed to providing ongoing support to family members for as long as needed.
